Demographic Data Transmittal Form

As a result of new U.S. Department of Education standards, the University of California is required to change the manner in which it collects and reports employee race and ethnicity data to the Integrated Postsecondary Education Data System (IPEDS).

In order to comply with the new requirements, the Demographic Data Transmittal (DDT) form U5605 and the Payroll Personnel System (PPS) have been modified.

Employees hired from August 1, 2010, forward should complete the revised form U5605 in order for the University to meet the new requirements and maintain the current reporting demands for campus affirmative action programs.

PLEASE NOTE: The revised DDT form is only for new hires from August 1, 2010 forward. Current employees do NOT need to fill out the revised form.

Changes to the Demographic Data Transmittal Form

Section 1 -- Race & Ethnicity
Section 1A collects the information as required by IPEDS.  The Department of Education requires educational institutions to use a two-part question format. The first question is to identify whether the respondent is Hispanic or Latino. The second question asks the respondent to select one or more races with which he or she identifies.  

Section 1B preserves the collection of the information for Affirmative Action program reporting and allows for a single race and ethnicity selection.

Section 2 – Disability
This section collects disability information.

Section 3 – Veteran Status
This section collects responses for individuals who wish to self-identify their veteran status.
